We would like to welcome you to our Aged and Community Care service. We understand the reluctance of some families, particularly in the Aboriginal community, to have someone else take care of Elders who are the most respected people in the community hierarchy.
Here at Biripi ACMC Aged and Community Care we are committed to providing culturally appropriate, quality and safe care delivery that promotes functional independence. We sincerely hope that we will be able to work in partnership with you to provide you with safe and quality care and services that will enable you to continue to live in your own home and environment within the community.
Our Home Care Packages are delivered within the Consumer Directed Care (CDC) framework. This means that you (and your carer) have more choices and control over what services are delivered, where and when they are delivered. Be rest assured that our staff and carers work hard to ensure each Elder is accorded the respect and dignity they deserve.

OUR SERVICES
Home Care Package (HCP)
The Home Care Package is funded by the Australian Government to support older Australians with receiving care and support to live safely and independently at home or their chosen environment for as long as possible. Home Care Packages are for older people requiring care and support due to being frail, have dementia or other special needs.
To be eligible for a Home Care Package you will be 50 years and above (Aboriginal and Torres Strait Islander) and 65 years and above (non-indigenous). You will also have to complete an online referral on My Aged Care (MAC) website. A staff member from Biripi Aged and Community Care can assist you with this process if requested.

Once approved, you are allocated a Home Care Package. Our skilled staff will work with you to identify your needs and goals in order to ensure choice and flexibility in choosing the services and support you need. There are four levels of home care packages available:
- Level 1 for people with basic care needs
- Level 2 for people with low-level care needs
- Level 3 for people with intermediate care needs
- Level 4 for people with high-level care needs
Biripi ACMC Aged and Community Care offers a range of flexible home care support options that can be tailored to suit your care requirements. For our Elders, our home care services are designed to promote functional independence so the Elders can remain the valuable assets within our communities.
We have a team of experienced staff including support workers who can deliver services and advice to ensure that your Home Care Package suits your lifestyle, personal goals and budget.
If you have a Home Care Package or Home Care Plan through My Aged Care system, we invite you to contact us at Biripi ACMC Aged and Community Care. We provide the following services and more under a Home Care Package:
- Personal Care
- Domestic Assistance including Home cleaning
- Shopping
- Nutrition and meal preparation
- Gardening and lawn-mowing
- Home Maintenance
- General wellbeing checks
- Companionship and socialisation
- Mobility and dexterity assistance
- Nursing & Allied health services
- Transport to and from social activities
- Transport to and from Allied health and medical appointments.
- Procurement of mobility aids and equipment
- Centre based respite
- In-Home care
Commonwealth Home Support Program (CHSP)
Commonwealth Home Support Program (CHSP) is funded by the Australian Government and provide entry-level home support care for frail older Australians requiring assistance to continue to live independently in their own homes and chosen environment for as long as possible.
Eligibility for CHSP is decided by a Regional Assessment Services (RAS) following care need assessments.
Services provided under the CHSP include:
- Centre based activities: Arts and Crafts, Exercises, or as shown on the activities calendar
- Centre based respite
- Overnight respite care
- Outings and day trips
- Group activities: Accompanying to appointments and support with phone calls
- Assisted transport to and from social activities
- Advocacy
- Communication with carers where appropriate
- Peer support
